re: the chicken whistle
There was a guild member next to the storehouse in the moon town who gave it to me. I was in the process of opening all the storehouses while trying to figure out how to use the chicken squares. I don't know if anything triggered it, I hadn't opened all the storehouses yet but I did have all the locations, so maybe that's it? Maybe it just happens w/o another trigger.
Two locations:
I assume that you've read the signs on the Level X Islands and gotten them into your list of locations. The last two are probably the Dead Lands and Dead Sea. Look for two more sign-posts. Dead Sea is on the west side of the eastern continent (there's an island w/the sign, but the actual area is the water near it, it'll be obvious). Dead Lands is in the mountains in the south of the Western continent, there's actually two clearings in the mountains w/ a sign, but they're both the Dead Lands and you only need one.

When using the warp function (V key on PC), you can switch between the Terra and Luna world maps (Z key on PC), and there's only town on Luna (mandatory visit during the main storyline), so you probably searched the whole time on Terra.
Thanks @karpfe for telling us the location of the last tool. This tool is incredibly easy to miss for people who do everything (like opening storehouses) as soon as possible. I thought the whole time the last tool would be located in one of the "post-game" dungeons, and I was on the verge of combing through every town/area again when I read this thread by chance. It seems the trigger for the last tool to appear is simply the "post-game" status (having beaten the game once) and nothing else.

The Catalog areas at the end are (from top to bottom): Energi Park, Dead Lands, Dead Seas, Arena, Hidden Arena, Other, Dogenkasen Tower. The Arena is located at the Guild (building in upper right corner). In the same building, you can also find the Hidden Arena (stairs in upper left corner), but only on the true ending route ("post-game"). If it's not one of the areas mentioned above that you're missing, please specify between which two areas in the Catalog your missing area is situated.
